Required Form to Report an Injury or Medical Incident

A Report of Occurrence must be submitted for injuries or medical incidents involving USA Swimming members or non-members occurring during a USA Swimming activity. See the report instructions for the timeframe in which the report must be submitted (differs depending upon the seriousness of the injury or incident).

Record Application Forms

If a National Age Group, American/U.S. Open, or World Record is set during a meet, use the applicable record application form to submit the required contact information and documentation.
